feat(notify): unify public-route error envelope and add push route tests

- all public write routes now return {"error":code,"request_id":id};
  retryable 503 responses add Retry-After: 30; 204 and success bodies unchanged
- GET /ready keeps its structured diagnostic JSON (Uptime Kuma runtime contract)
- add HTTP tests for the push register/disable routes
